Start with the County Clerk's office. The case would have been brought in your name or one of your parents. You can get a copy of the court order approving the settlement. That will give you the name of the attorney. Call the attorney and ask what bank the the trust account was set up in (that might be in the court order as well). This will give you the information about the terms of the settlement and when you can have access to your funds.

Generally, if your harm was caused by another person's negligence (and the therapist was not a public employee) the statute of limitations is three years. There are various exceptions. I don't know where you found the four-year period, but it's not in my statute book for Wisconsin. (There is only one four-year limitations period I know of, and that has to do with fraudulent conveyances, very different from your complaint.) Of course, the longer you wait, even within the time period, the harder it is to prove your damages. You should certainly see a physician promptly after you discover the harm you suffered, and document it every way you can.
