The steps are whatever the DMV and court records say they are, regardless of what anyone tells you otherwise. Since you can't get answers yourself, you may be forced to hire counsel to help you.

Most likely there is a probation violation and a bench warrant issued for her failure to follow through. Although an attorney cannot guarantee that she will not be taken into custody, if she turns herself in, (s)he can at least try to negotiate a resolution prior to your niece appearing in court. If she cannot afford an attorney, you or she may want to contact the local public defender's office and see if they are willing to help.

I don't know whether or not this is an ethical violation reportable to the state bar, but you may want to speak to this P.D.'s supervisor regarding the allegations.
