You seem to be off on the dates, because your 2009 return wouldn't have been filed before early 2010. However, assuming there was no fraud or other problems, taxes that were assessed on returns filed 3 or more years prior to the bankruptcy are generally dischargeable in Chapter 7. The code language is complex and confusing (see 11 U.S.C. section 523). Best to consult with an experienced bankruptcy attorney before filing.

You can get a book at the bookstore for bankruptcy, such as those by Nolo, that might help you file yourself. Also, the bankruptcy court will generally let you make installment payments towards the filing fee. If you have any concerns about your bankruptcy, you may want to hire a lawyer to at least review your case.

Creditors need court permission to contact you after you file bankruptcy. However, I always advise my clients to allow time for the creditors to receive notification and update their system before getting too upset about the creditors contacting them after the case is filed. However, if the creditor is aware of the bankruptcy and chooses to ignore the restriction on contacting the debtor, then the debtor can file an adversary proceeding against the creditor.
